European Co-Care Learning Ecosystems (Co-Care)

CoCare
project’s aim is to provide participatory and evidence-based pathways for a
paradigm shift towards a more equitable, integrated and accessible healthcare
systems in primary and community care practices and policies. Based on the evidence
generated through national cases in five European countries, a framework for
European co-care learning ecosystems is developed, encompassing policy
recommendations, best practices, and strategies for fostering regulatory reforms,
changes in funding models, and the development of new infrastructure and
technology to support integrated care.

Project description

University of Vaasa (Finland), Karolinska Institutet (Sweden), University of Lucerne (Switzerland), University of Limoges (France) and I.R.C.C.S. Ospedale Galeazzi - Sant'Ambrogio (OGSA) (Italy) foster a culture of co-creation and co-learning. Through a series of participatory workshops, conferences, and other dialog-embedded activities, collaboration and knowledge exchange among health and care providers, policymakers, researchers, community organizations, and end-users, the CoCare project contributes to the development of European co-care learning ecosystem.
